Tribal Jurisdiction Sample Progress Reporting Form

View this sample reporting form to better understand reporting requirements for OVW grant-funded activities.

OVW grant recipients are statutorily required to report on grant-funded activities every six months. The progress report is due within 30 days of the end of the current reporting period.  For the reporting period from January 1 – June 30, the deadline is July 30; for the reporting period from July 1 – December 31, the deadline is January 30.

Tribal Jurisdiction grantees complete their progress report online, via SurveyMonkey, and then submit a PDF of their SurveyMonkey progress report into their JustGrants account.

All Tribal Jurisdiction grantees receive an email from VAWA MEI with a link to their SurveyMonkey progress report approximately one month before the report is due.

Please note: This sample form is for reference purposes only! It can be downloaded to see what questions will be asked in the SurveyMonkey report. But this sample form cannot be filled out or submitted in JustGrants. All grantees must submit their report through their SurveyMonkey link.

About the reporting process

There are three steps in the reporting process.

  1. Each grantee will complete and submit their progress report in SurveyMonkey, using the link that was sent to their email account.  This step includes uploading a separate PDF/Word document for Question 71 (see “About Question 71” below).
  2. Once a grantee has submitted their progress report in SurveyMonkey, they will receive an email with a PDF of their report within one business day.
  3. Each grantee must upload the PDF of their report into their JustGrants account.  Please note that only the person identified as the Grant Award Administrator in JustGrants can upload progress reports. This step must be completed by the report due date.

About Question 71

Question 71 (“Progress toward project goals”) requires each grantee to upload a separate document listing their project’s goals and objectives and their progress toward them, as a Word or PDF file. The progress report is not complete without this document.
